Sinopse: A enteada de Johan Falk, Nina, decide passar mais tempo com Orjan. Orjan está usando os serviços ilegais oferecidos a ele por uma desonesta empresa de construção. Quando ele se vê incapaz de pagar pelos serviços, ele coloca Nina em perigo ocasionando seu sequestro. Para achar Nina, Johan envolve o GSI, criando uma bola de neve que os leva à máfia mais perigosa da Europa, a Karayan.

Depois dos três primeiros aterrorizantes ataques dos vermes malditos, finalmente saberemos agora como tudo começou. A ação começa no século 19, quando 17 trabalhadores morrem misteriosamente em uma mina. Apavorados, os habitantes de Rejection abandonam a cidade. Com a mina fechada, quem perdeu tudo e não tem para onde ir discute o futuro de suas vidas. É neste cenário de medo e desolação que Juan Perrila (Brent Roam), um dos sobreviventes, negocia com Hiram Gummer (Michael Gross), o proprietário da mina. Ele aceita voltar ao trabalho, desde que o seu salário seja dobrado. Porém, Juan não sabe que pode estar negociando a própria vida: os vermes malditos continuam à solta, esperando para devorar mais e mais mineiros.

John Falk is on a routine mission with the Gothenburg police special unit GSI. With the help of a Norwegian undercover police and a civilian infiltrator in one of Gothenburg's heaviest criminal gangs, the GSI are trying buy six assault rifles and thus get them off the street. But suddenly everything changes when a new unknown player suddenly and immediately wants to buy not only those weapons but even more, including armor-piercing shells. GSI realizes that this can not be any ordinary robbers. Someone will start a war on the streets of Gothenburg. At the same time as the situation gets more and more complicated for John Falk's infiltrator Frank Wagner when the leader of the gang he infiltrated realize that one of his own leaking to the police.

After causing a commotion with his last assignment, Falk has been given a desk job, which hardly agrees with his personality, and he ends up accepting an offer from an old friend to buy into a private investigation agency. Another friend of Falk's, Sven, purchased a business in Estonia, and when local gangsters attempted to pressure him into paying protection money, Sven retained the services of Nikolaus Lehmann, a burly private eye, to throw them off his trail. However, Lehmann does his job all too well, murdering the racketeers, and then threatening Sven and his family. With no where else to turn, Sven asks Falk to help him deal with the crazed Lehmann; Falk agrees, but soon realizes he's dealing with a more dangerous man than he imagined when Lehmann kidnaps Falk's wife Jeanette, and then releases her with a time bomb locked around her neck, demanding that Falk hand over ownership of his detective agency to Lehmann.
